What is Chronic Obstructive Pulmonary Disease?

Chronic Obstructive Pulmonary Disease (COPD) is a chronic, progressive condition that causes breathing problems. The term “progressive” means the disease tends to worsen over time.

COPD can present with symptoms such as:

  • coughing with heavy mucus production
  • shortness of breath
  • breathlessness
  • a sense of tightness in the chest

It is important to note, however, that many patients experience no noticeable symptoms until the disease reaches more advanced stages.

Cigarette smoking is the leading cause of the condition, and most patients with COPD are current or former smokers. Prolonged exposure to other lung irritants, such as air pollutants, chemical fumes, and dust, can also contribute to the development of COPD.

To better understand COPD, it helps to first understand how the lungs work. The air we breathe travels through the trachea and reaches the bronchi, two tubes located within the lungs.

Inside the lungs, the bronchi branch into thousands of smaller, thinner tubes called bronchioles, which end in tiny hemispherical air sacs called alveoli.

The alveoli are surrounded by small blood vessels called capillaries. When air reaches the alveoli, oxygen passes through their walls into the blood flowing through the capillaries. At the same time, carbon dioxide (a waste gas) moves in the opposite direction, from the capillaries into the alveoli. This process is known as gas exchange and forms the basis of breathing.

The bronchi and alveoli are elastic:

  • when we inhale, the alveoli fill with air, like tiny balloons
  • when we exhale, the alveoli deflate and air is expelled

In patients with COPD, airflow through the respiratory tract is reduced due to a variable combination of the following factors:

  • the bronchi and alveoli lose their elasticity
  • many of the walls separating the alveoli are destroyed
  • the walls of the bronchi thicken and become inflamed
  • the bronchi produce more mucus (phlegm) than normal, which tends to clog the airways

Causes

Most cases of COPD result from prolonged exposure to irritants that damage both the lungs and the bronchi, typically cigarette smoke, though pipe smoke, cigars, and other forms of tobacco use can also cause COPD, especially when the smoke is inhaled.

Smoking accounts for roughly nine out of ten cases, and smokers are about 13 times more likely to die from COPD than non-smokers.

The development of COPD can also be promoted by:

  • secondhand smoke (smoke inhaled while near someone else who is smoking)
  • air pollution
  • chemical fumes
  • environmental or occupational dust exposure
  • a childhood marked by frequent respiratory infections

In some patients, a genetic condition called alpha-1 antitrypsin deficiency may also play a significant role. People with this condition have low levels of alpha-1 antitrypsin (AAT), a protein produced by the liver. This deficiency increases the risk of lung damage and COPD if the person is exposed to smoke or other lung irritants. If you have this condition and smoke, COPD can progress very quickly.

Risk Factors

The primary risk factor for developing COPD is, unsurprisingly, smoking. Most patients with COPD are current or former smokers. Those with a family history of the disease face an even higher risk if they smoke.

Most patients with COPD are over the age of 40 when symptoms first appear. COPD rarely affects people under 40, except in cases involving the genetic condition alpha-1 antitrypsin deficiency.

COPD is not contagious.


Symptoms

The main signs and symptoms of COPD include:

  • chronic cough or a cough that produces large amounts of phlegm (often referred to as “smoker’s cough”)
  • shortness of breath (dyspnea), especially during physical activity
  • wheezing while breathing
  • increased frequency of respiratory infections
  • cyanosis (bluish fingers and lips)
  • fatigue
  • a tight sensation in the chest

These symptoms often appear many years before airflow through the lungs begins to noticeably decline. However:

  • not everyone with these symptoms has COPD
  • not every COPD patient experiences all these symptoms

If you have COPD, you’re likely to catch colds or the flu more often than healthy individuals. In severe COPD, you may experience swelling in the ankles, feet, or legs (edema), bluish lips (due to very low oxygen levels), and persistent breathlessness.

COPD symptoms usually worsen gradually. In the early stages, they are often mild enough to go unnoticed, or easily managed with small lifestyle adjustments to breathe more comfortably, such as taking the elevator instead of the stairs.

Over time, symptoms can become severe enough to prompt a visit to the doctor, for example, if breathlessness starts occurring after only light physical exertion.

The severity of symptoms depends on the extent of lung damage. If smoking continues, this damage will progress more rapidly.

In advanced COPD, additional symptoms may develop, such as unintentional weight loss and reduced muscle endurance.

In some severe cases, hospitalization may be required.


When to Call a Doctor

Seek emergency care immediately, or have someone take you if you’re unable to, if you experience:

  • Severe difficulty breathing or speaking
  • Bluish or grayish lips or fingertips (cyanosis, a sign of low blood oxygen)
  • Inability to stay alert (lethargy)
  • A rapidly beating heart (tachycardia)
  • No improvement despite following your prescribed treatment for worsening symptoms

Dangers and Complications

COPD is one of the leading causes of disability and ranks as the third leading cause of death worldwide. Experts also believe many more people may have the disease without knowing it.

COPD develops slowly and insidiously, with symptoms that often worsen over time and can limit daily activities. In severe cases, it can prevent patients from performing even basic tasks such as walking, cooking, or caring for themselves.

There is currently no definitive cure for COPD, and doctors cannot fully restore lung tissue that has already been destroyed. However, current treatments and lifestyle changes can help patients feel better, stay more active, and significantly slow the progression of the disease.


Diagnosis

Diagnosing COPD requires an integrated approach that combines the patient’s clinical history with objective lung function tests. Clinical suspicion typically arises when persistent symptoms such as dyspnea, chronic cough, or sputum production are present, especially in individuals with a history of exposure to risk factors like smoking or environmental pollutants.

Spirometry: The Gold Standard

The essential test for confirming a COPD diagnosis is spirometry. This test measures the volume of air a person can exhale and how quickly they can do so. A COPD diagnosis is confirmed if, after administering a bronchodilator, the ratio between forced expiratory volume in one second (FEV1) and forced vital capacity (FVC) is below 0.70. This value indicates persistent airflow obstruction.

Multidimensional Assessment (GOLD Classification)

Once airflow obstruction is confirmed, the doctor doesn’t rely solely on the spirometry numbers, but performs a comprehensive assessment to define the best treatment strategy. This process includes:

  • Symptom assessment: Validated questionnaires such as the CAT (COPD Assessment Test) or the mMRC (Modified Medical Research Council) scale are used to measure the disease’s impact on daily life.

  • History of exacerbations: The number of acute flare-ups experienced in the past year is reviewed, since patients with frequent or severe episodes (requiring hospitalization) are at higher risk of disease progression.

  • Phenotyping: A simple blood test measures eosinophil count. This biomarker helps predict whether a patient is likely to respond well to corticosteroid therapy.

Imaging and Other Diagnostic Tests

To complete the clinical picture and rule out other conditions such as asthma or heart failure, additional tests may be needed:

  • Chest X-ray: Useful for ruling out complications or alternative diagnoses, though not sufficient on its own to diagnose COPD in its early stages.

  • High-resolution CT scan of the chest: Essential for identifying emphysema and, in select cases, for early lung cancer screening, since smoking is a shared risk factor between COPD and lung cancer.

  • Arterial blood gas analysis: Reserved for patients with severe symptoms or low oxygen saturation, to precisely measure blood oxygen and carbon dioxide levels.

  • Alpha-1 Antitrypsin testing: Recommended at least once for all patients diagnosed with COPD, particularly younger patients or non-smokers, to rule out a genetic predisposition.


Treatment and Therapy

The main goals of COPD management are to reduce symptoms (by improving exercise tolerance and overall health status) and to reduce future risks, particularly by preventing exacerbations and slowing the decline in lung function. Today’s treatment plans are highly personalized based on each patient’s profile.

Inhaled Pharmacological Therapy

The inhaled route is preferred because it maximizes effectiveness in the lungs while minimizing systemic side effects. Key medications include:

  • Long-acting bronchodilators (LABAs and LAMAs): These form the foundation of therapy. LAMAs (long-acting antimuscarinics) and LABAs (long-acting beta2-agonists) relax the muscles surrounding the airways. Combining both is often more effective than either alone at preventing exacerbations.

  • Triple therapy: For patients who continue experiencing exacerbations despite treatment, or who have high blood eosinophil levels, a combination of three medications in a single inhaler (LAMA + LABA + ICS, an inhaled corticosteroid) is used. This strategy has been shown to significantly reduce mortality and hospitalizations.

  • Rescue medications: Fast-acting bronchodilators are prescribed for immediate relief during acute episodes of breathlessness.

New Frontiers: Biologic Therapies

For a specific subgroup of patients with severe COPD and signs of type 2 inflammation (characterized by elevated blood eosinophil levels), targeted biologic medications are now available. These drugs act on the underlying biological mechanisms of inflammation, dramatically reducing the frequency of respiratory flare-ups when standard inhaled medications are not enough.

Managing Flare-Ups and Vaccinations

Exacerbations are critical events that can cause permanent lung damage. Prevention also relies on appropriate vaccination. Current guidelines recommend:

  • Annual flu vaccination
  • Pneumococcal vaccination (against bacterial pneumonia)
  • Vaccination against Shingles (Herpes Zoster), COVID-19, and Respiratory Syncytial Virus (RSV) for individuals over 60 or with specific vulnerabilities

Pulmonary Rehabilitation and Lifestyle

Pulmonary rehabilitation is considered one of the most effective treatments available. It’s a multidisciplinary program that includes:

  • Physical training: Targeted exercises to improve muscle strength and cardiovascular capacity, reducing the perception of breathlessness during daily physical activity.

  • Education and nutrition: Dietary guidance to prevent muscle mass loss or manage excess weight, both of which can complicate breathing.

  • Smoking cessation: This remains, by far, the most effective intervention available. Even in advanced stages, quitting smoking slows the decline in lung function. Medications such as varenicline or nicotine replacement therapy may be prescribed to support cessation.

Oxygen Therapy and Ventilatory Support

For patients with chronic respiratory failure, long-term oxygen therapy (at least 15 hours a day) is recommended to improve survival and protect the heart. In some patients who experience excessive carbon dioxide buildup during sleep, nighttime non-invasive mechanical ventilation (NIV) via a small facial mask may be beneficial.

Surgical and Endoscopic Interventions

For selected patients with severe emphysema, options exist to reduce the volume of the most damaged lung tissue, allowing healthier areas to function more effectively:

  • Endoscopic lung volume reduction: A minimally invasive procedure involving the placement of one-way valves into the airways via bronchoscopy.

  • Surgical lung volume reduction or lung transplant: Reserved for very specific cases at specialized centers, following careful evaluation of the risks and benefits.

Preparing for Emergencies

Patients with COPD should have a personalized “action plan” developed together with their pulmonologist. This plan should clearly outline when to increase inhaled medication doses, when to start oral antibiotics or corticosteroids, and when to seek emergency care (for example, in cases of mental confusion, cyanosis, or breathlessness at rest that doesn’t respond to medication).


Prevention

There are steps you can take to help prevent COPD before it develops. If you already have the condition, you can still take action to prevent complications and slow its progression.

Preventing COPD Before It Develops

  • The best way to prevent COPD is to never start smoking, or to quit if you already do. Smoking is, by far, the leading cause of COPD.

  • Try to avoid other irritants that can contribute to COPD, such as secondhand smoke, air pollution, chemical fumes, and dust (secondhand smoke refers to smoke inhaled when others around you are smoking).

Preventing Complications and Slowing Disease Progression

  • If you have COPD, the single most important step you can take is to quit smoking. Doing so can help prevent complications and slow the progression of the disease. It’s also wise to avoid exposure to the irritants listed above.

  • Follow your prescribed COPD treatment plan carefully. Doing so will help you breathe more easily, stay active, and avoid or better manage severe symptoms.

  • Ask your doctor whether and when you should get vaccinated against the flu and pneumonia. Vaccination reduces the risk of these illnesses, which pose serious health risks for people living with COPD.
