What Is Crohn’s Disease?
Crohn’s disease is a chronic inflammatory condition of the gastrointestinal (GI) tract. It most commonly affects the terminal ileum, which is the final section of the small intestine, and the beginning of the large intestine (colon). However, it can involve any segment of the digestive tract, ranging from the mouth to the anus.
Crohn’s disease belongs to the broader category of Inflammatory Bowel Diseases (IBD), which also includes ulcerative colitis and microscopic colitis.
A defining feature of Crohn’s disease is that inflammation may affect the full thickness of the bowel wall and can be discontinuous. This means patches of inflamed tissue may appear alongside healthy tissue, which are often called “skip lesions”. This distinguishes it from ulcerative colitis, where inflammation is typically continuous and confined to the innermost lining (mucosa).
The disease generally develops gradually and tends to worsen over time, though it is characterized by alternating periods of active disease (flares) and remission, which can last weeks, months, or occasionally years.
Because the inflammation can vary so widely in extent and depth, early symptoms are often vaguer than those of ulcerative colitis, presenting primarily as diarrhea (with or without blood) and abdominal pain. This is why many patients experience symptoms for months or even years before a definitive diagnosis is established.
Crohn’s disease is currently considered a chronic condition without a definitive cure. However, modern therapies can effectively control symptoms, prevent complications, and achieve complete clinical and histological remission in a significant proportion of patients.
Causes
The exact causes of Crohn’s disease remain unknown. The prevailing scientific consensus is that it results from a complex interplay of immune, genetic, microbial, and environmental factors.
The dominant hypothesis is that Crohn’s disease stems from an abnormal autoimmune response. In this scenario, the immune system mistakenly attacks the body’s own intestinal cells. This reaction may be triggered by intestinal bacteria, viruses, or other environmental agents. This dysregulated immune response drives chronic intestinal inflammation.
Prevalence
It is estimated that more than 500,000 people in the United States are affected by Crohn’s disease.
Scientific studies have demonstrated a progressive increase in disease incidence over recent decades in both North America and Europe. This trend is also evident in countries undergoing rapid “Westernization” of lifestyle, though the reasons for it remain the subject of active investigation.
Risk Factors

Family history: Crohn’s disease frequently runs in families. Having a first-degree relative (parent or sibling) with IBD significantly increases personal risk. The genetic architecture of inflammatory bowel disease is polygenic; more than 240 genetic loci have been linked to its susceptibility.

Smoking: One of the most important modifiable risk factors. Smoking is associated with approximately double the risk of developing Crohn’s disease compared to non-smokers, and significantly worsens disease course, increases the need for surgery, and reduces response to therapy.

Medications: Certain drugs appear to modestly increase risk, including NSAIDs (non-steroidal anti-inflammatory drugs) such as aspirin and ibuprofen, as well as antibiotics and oral contraceptives.

Diet: A diet high in animal fats and processed foods, and low in fruits and vegetables, is associated with modestly increased incidence.

Westernization: A landmark review published in The Lancet analyzed nearly 200 studies. The findings revealed a strong correlation between IBD incidence and the “Westernization” of societies. This process involves not only dietary patterns but also urbanization, reduced microbial diversity, and changes in early-life microbiome exposure.
Important: Stress and specific foods do not cause Crohn’s disease, but both can trigger flares and worsen existing symptoms.
Symptoms
Crohn’s disease can affect people of all ages, but symptoms most commonly begin in childhood or early adulthood. In fact, the majority of diagnoses are established before the age of 30. The clinical presentation varies considerably from patient to patient and across time in the same patient.
Onset is typically gradual and nonspecific (abdominal pain and diarrhea), but in some patients it can be sudden and dramatic. A subset of patients is able to identify specific foods that consistently trigger or worsen their symptoms.
Most Common Symptoms
- Abdominal cramps and pain, which often worsen after meals
- Diarrhea, which can be watery or more formed, may occur up to 20 times per day. It can even wake the patient from sleep, a key difference from irritable bowel syndrome, where nocturnal symptoms do not occur
- Blood in the stool: this may appear bright red or darker in color and is not always present
- Pus and/or mucus in the stool
- Flatulence and abdominal bloating
- Fatigue, often profound and debilitating
- Unexplained weight loss
Other Symptoms
Crohn’s disease is a systemic disease, meaning it can cause manifestations far beyond the gut.
- Anemia, which can result from chronic blood loss, iron deficiency, or vitamin B12 malabsorption
- Mouth ulcers (aphthous stomatitis)
- Uveitis: eye inflammation that causes redness, pain, and photophobia
- Fever
- Joint pain (arthralgia/arthritis)
- Nausea or loss of appetite
- Skin manifestations include erythema nodosum, which are tender red nodules typically found on the shins, and pyoderma gangrenosum, ulcerating skin lesions
Classification of Symptoms by Severity
Symptoms vary depending on the site and depth of inflammation. They typically follow a relapsing-remitting pattern: periods of complete or near-complete remission can last weeks to months, interrupted by flares of varying intensity.
International guidelines classify disease activity as follows:

Stress and Crohn’s Disease
The relationship between psychological stress and IBD is bidirectional and clinically important. Studies suggest that stress, including the stress of living with a chronic disease, can worsen symptom burden and trigger flares. Conversely, research has demonstrated that depression can worsen Crohn’s disease, making it more aggressive. At the same time, living with IBD itself significantly increases the risk of depression and anxiety. Data from the Sinai-Helmsley Alliance for Research Excellence (SHARE) cohort indicate that approximately 1 in 5 patients with Crohn’s disease develops clinical depression.
Differential Diagnosis
Crohn’s disease must be differentiated from other conditions, most importantly ulcerative colitis. Key distinguishing features:

Complications
Intestinal Complications

Intestinal obstruction: Chronic inflammation causes progressive thickening and scarring of the bowel wall, which can narrow the intestinal lumen (stricture), partially or completely blocking the passage of food and stool.

Fistulas: When transmural inflammation penetrates the entire thickness of the intestinal wall, it forms abnormal tunneling connections. These connections, known as fistulas, can develop between loops of bowel, between the bowel and other organs like the bladder, vagina, or skin, or between the bowel and the external surface. Fistulas can become infected.

Abscesses: Localized collections of pus within the abdominal cavity or perianal region, resulting from deep penetrating inflammation or infected fistulas. Abscesses are painful and may require surgical drainage.

Anal fissures: Small tears in the anal canal that cause pain, itching, and bleeding. They are particularly common in Crohn’s disease because of perianal involvement.

Ulcers: Open sores (ulcerations) can form anywhere along the GI tract, including the mouth, small intestine, large intestine, anus, and perineum.

Malnutrition: Chronic inflammation, diarrhea, malabsorption, and reduced appetite collectively impair the absorption of essential nutrients. These include iron, vitamin B12, folate, calcium, and vitamin D. As a result, nutritional deficiencies can develop.

Extra-intestinal inflammation: The joints (arthritis), eyes (uveitis, episcleritis), liver (primary sclerosing cholangitis), and skin (erythema nodosum, pyoderma gangrenosum) can all be affected.
Conditions Associated with Crohn’s Disease
When Crohn’s disease involves the colon, patients face a moderately increased risk of colorectal cancer compared to the general population. The risk increases with longer disease duration and greater extent of colonic involvement.
Achieving and maintaining mucosal remission through appropriate treatment can reduce this cancer risk.
Patients with colonic Crohn’s disease should discuss colorectal cancer screening with their gastroenterologist. Surveillance colonoscopy with systematic biopsies is typically recommended at regular intervals. The frequency depends on individual risk factors, such as disease duration, extent, and the presence of primary sclerosing cholangitis. Screening does not reduce the probability of developing cancer per se, but enables early detection when cure rates are highest.
Diagnosis
The diagnostic workup for Crohn’s disease is now oriented not merely toward confirming the diagnosis, but also toward stratifying disease risk and precisely mapping the extent and severity of intestinal damage. Early, accurate diagnosis is essential to prevent irreversible complications such as strictures, fistulas, and bowel damage.
Medical History and Physical Examination
The physician begins with a comprehensive clinical assessment:
- Medical history: duration and character of bowel symptoms (particularly >4 weeks of diarrhea), family history of IBD, smoking status, medication use, and any extra-intestinal manifestations.
- Physical examination: abdominal palpation for tenderness, masses, or distension; careful perianal inspection for fissures, skin tags, fistula openings, or abscesses. Perianal involvement is highly suggestive of Crohn’s disease.
Laboratory Tests and Biomarkers
Modern diagnostics use increasingly precise tools to differentiate IBD from functional bowel disorders:

Fecal calprotectin: Currently the most important first-line biomarker. Elevated levels of this neutrophil-derived protein in the stool confirm the presence of active intestinal inflammation and guide the need for further endoscopic investigation. It is non-invasive, inexpensive, and highly sensitive for IBD.

Blood tests:
- Inflammatory markers: C-Reactive Protein (CRP) and Erythrocyte Sedimentation Rate (ESR), which are elevated in active disease.
- Complete blood count (CBC): assessing for anemia (red blood cells, white blood cells).
- Nutritional parameters: serum albumin, iron, ferritin, vitamin B12, folate, and vitamin D. These tests help identify malabsorption and deficiencies.

Stool cultures and microscopy: Essential to exclude infectious causes, such as Clostridioides difficile, Campylobacter, Salmonella, Shigella, Yersinia, and parasites. These infections can closely mimic IBD and must be ruled out before initiating immunosuppressive therapy.

ASCA and ANCA antibodies: Serological markers (anti-Saccharomyces cerevisiae antibodies and perinuclear anti-neutrophil cytoplasmic antibodies) can aid in differentiating Crohn’s disease from ulcerative colitis in ambiguous cases, though they are not diagnostic in isolation.
Imaging Studies
Because Crohn’s disease can involve segments of the small intestine inaccessible to standard colonoscopy, dedicated imaging is critical:

Intestinal ultrasound (IUS): A non-invasive, radiation-free tool for assessing bowel wall thickness, vascularity, and detecting complications such as abscesses. Increasingly used for disease monitoring and response assessment.

Magnetic Resonance Enterography (MR enterography / MRE): Currently the gold standard for small bowel assessment. Provides detailed visualization of the bowel wall, mesentery, and adjacent structures. It identifies active inflammation, fibrotic strictures, and fistulas. Importantly, it involves no ionizing radiation, which is especially valuable for young patients who require repeated imaging.

CT scan (TAC): Preferred in emergency settings to rapidly evaluate suspected complications such as acute bowel obstruction, perforation, or intra-abdominal abscesses.
Endoscopic Procedures
Ileocolonoscopy with multiple biopsies remains the cornerstone of diagnosis, providing direct mucosal visualization and histological confirmation:

Allows direct inspection of the mucosa from the rectum to the terminal ileum

Tissue biopsies confirm the diagnosis histologically and differentiate Crohn’s disease from ulcerative colitis and other conditions

Characteristic endoscopic findings include: aphthous ulcers, cobblestone mucosal pattern, deep longitudinal ulcers, and skip lesions

Upper GI endoscopy (gastroscopy): Recommended when upper GI symptoms (dysphagia, epigastric pain) are present or when upper tract involvement is clinically suspected

Capsule endoscopy (video capsule): A swallowable wireless camera that images the entire small intestine. Indicated when clinical suspicion remains high but standard investigations are inconclusive. Contraindicated in the presence of suspected strictures due to retention risk.
Treatment
The therapeutic approach to Crohn’s disease has undergone a paradigm shift toward precision medicine. The goal is no longer simply symptom control, or clinical remission. Instead, it focuses on achieving mucosal healing: the complete resolution of intestinal inflammation, confirmed both endoscopically and histologically. This approach helps prevent irreversible structural damage and reduces the need for future surgery. This strategy is known as “treat-to-target” (T2T).
Treatment Strategies
Available options include biological therapies, innovative oral agents, nutritional support, and conservative surgery. Treatment selection is guided by disease severity at diagnosis, location and extent of disease, risk stratification, and individual patient factors.
Pharmacological Therapies
Corticosteroids
Prednisone and methylprednisolone are highly effective for rapidly inducing remission during acute flares. However, because of significant long-term side effects like osteoporosis, adrenal suppression, and metabolic effects, corticosteroids are used only for short-term induction. They are not appropriate for maintenance therapy.
Budesonide (an oral corticosteroid with predominantly local intestinal action and reduced systemic absorption) is preferred for ileocecal disease, offering effective flare control with a more favorable side effect profile.
Note: Mesalazine (5-ASA) and aminosalicylates, while commonly used in ulcerative colitis, have a very limited role in Crohn’s disease due to inferior efficacy compared to other available options.
Traditional Immunosuppressants
- Azathioprine / 6-Mercaptopurine: Used primarily for maintenance of remission, often in combination with biological agents to optimize efficacy and prevent loss of response (immunogenicity)
- Methotrexate: An alternative immunosuppressant, particularly in patients intolerant to azathioprine; also used to maintain remission
Biological Agents and Small Molecules
These represent the cornerstone of modern Crohn’s disease treatment: monoclonal antibodies, which are engineered to target specific proteins that drive intestinal inflammation.

Anti-TNF agents (Infliximab, Adalimumab, Certolizumab pegol): The most widely used and longest-established biological class, with extensive evidence for inducing and maintaining remission and promoting mucosal healing. Infliximab is administered intravenously; adalimumab and certolizumab by subcutaneous injection.

Anti-integrin agents (Vedolizumab): Selectively block lymphocyte trafficking to the gut, providing gut-selective immunosuppression with a favorable systemic safety profile. Administered intravenously.

Anti-interleukin agents:
- Ustekinumab (anti-IL-12/23): Effective for moderate-to-severe Crohn’s, administered as a single intravenous induction dose followed by subcutaneous maintenance injections
- Risankizumab (anti-IL-23): Recently approved, demonstrating high efficacy particularly in patients who have failed anti-TNF therapy

JAK inhibitors (Upadacitinib): A newer class of oral small-molecule drugs (not injections) that inhibit intracellular signaling pathways driving inflammation. Offer a powerful option for patients who fail or are intolerant of biological therapy. Require monitoring for specific side effects including infections and cardiovascular events.
Nutrition and Lifestyle
Lifestyle modifications play a crucial role in managing Crohn’s disease. They are not optional add-ons, but essential parts of treatment.

Smoking cessation: The single most important non-pharmacological intervention. Smoking dramatically worsens disease course, increases complication rates, reduces response to biological therapy, and doubles post-surgical relapse risk. All smoking patients must be strongly and repeatedly counseled to quit.

Individualized dietary management: No single diet is universally recommended. However, during active flares, Exclusive Enteral Nutrition (EEN), a liquid formula diet that provides complete nutrition, can effectively induce remission, particularly in children. A low-fiber diet during flares reduces mechanical irritation. The specific carbohydrate diet and Mediterranean diet show promising results in emerging studies.

Nutritional supplementation: Iron infusions, vitamin B12, vitamin D, calcium, and folate supplementation are frequently required to correct malabsorption-related deficiencies.

Psychological support: Cognitive behavioral therapy (CBT), mindfulness, and peer support groups address the significant mental health burden of living with a chronic disease and have been shown to improve both psychological wellbeing and disease outcomes.
Conservative Surgery
Surgery in Crohn’s disease is no longer considered the “last resort”. Instead, it serves as a strategic therapeutic option that doctors use selectively. Modern techniques, particularly laparoscopy, have made procedures less invasive with faster recovery times.

Strictureplasty: Surgically widening narrowed intestinal segments (strictures) without removing bowel, preserving intestinal length and function. Indicated for fibrotic strictures that do not respond to medical therapy.

Ileocecal resection: Removal of the terminal ileum and cecum. Often highly effective for patients with localized ileocecal disease refractory to medical therapy. Post-operative prophylaxis with anti-TNF therapy is recommended to prevent endoscopic recurrence.

Management of complications: Surgical drainage of intra-abdominal abscesses; repair or resection of complex fistulas.

Intestinal stoma: In extreme cases, a temporary stoma (allowing the inflamed bowel to rest) or, rarely, a permanent stoma may be necessary. All surgical procedures are performed under general anesthesia using Enhanced Recovery After Surgery (ERAS) protocols to minimize recovery time.
Managing Day-to-Day Symptoms
For mild symptom control, physicians may recommend:
- Paracetamol (acetaminophen): This is the preferred option for pain relief. NSAIDs should be avoided, since they can trigger flares and directly irritate the intestinal mucosa.
- Loperamide: For managing profuse diarrhea, it should be used with caution and only for short periods. It is contraindicated in the presence of active severe colitis due to the risk of toxic megacolon.
- Antispasmodics: For cramping and abdominal pain relief.
Diet and Crohn’s Disease
There is no single proven dietary regimen that can treat or cure Crohn’s disease. However, dietary modifications can meaningfully help reduce symptoms in many patients, particularly during flares. Any dietary changes should be discussed with your physician or a registered dietitian with IBD expertise.
General Dietary Modifications That May Help
- Avoid carbonated and fizzy drinks, as they can worsen bloating and gas.
- Reduce high-fiber foods during flares. These include popcorn, raw vegetable skins, nuts, and seeds, as they can mechanically irritate the inflamed bowel.
- Stay well hydrated, as this is essential to compensate for fluid losses from diarrhea.
- Eat small, frequent meals rather than large ones. This helps reduce digestive burden and post-prandial pain.
- Keep a food diary to identify individual trigger foods that consistently worsen symptoms.
Specific Dietary Approaches (as Clinically Indicated)
Depending on symptoms, disease stage, and medications, the physician or dietitian may recommend:
- High-calorie diet: to address weight loss and malnutrition during active disease
- Lactose-free diet: particularly if secondary lactose intolerance (due to small bowel involvement) is contributing to symptoms
- Low-fat diet: to reduce diarrhea and steatorrhea (fatty stools) when fat malabsorption is present
- Low-fiber (low-residue) diet: during acute flares or in the presence of strictures, to minimize mechanical obstruction risk
- Low-sodium diet: may be recommended during corticosteroid therapy (which causes sodium and fluid retention)
Nutritional Supplementation
Many patients with Crohn’s disease require supplementation due to malabsorption. Your physician should assess for deficiencies in:
- Iron (corrected orally or intravenously depending on severity)
- Vitamin B12 (particularly when the terminal ileum is involved)
- Vitamin D and calcium (to protect bone health, since IBD and corticosteroid use both increase osteoporosis risk)
- Folate
- Zinc
Always consult your physician before using dietary supplements or complementary/alternative therapies, as some supplements can interact with IBD medications or trigger immune responses.
