Causes

Medications (The Most Common Cause)

Hundreds of pharmacological agents list dry mouth among their known side effects. In fact, medication-induced xerostomia is the single most common cause encountered in clinical practice. Particularly implicated drug classes include:

  • Antidepressants and anxiolytics: tricyclic antidepressants (TCAs), SSRIs, SNRIs, and benzodiazepines all reduce salivary flow through anticholinergic or sympathomimetic mechanisms
  • Antihistamines: both first-generation (sedating) and, to a lesser degree, second-generation antihistamines exert anticholinergic effects that inhibit salivary gland secretion
  • Decongestants: alpha-adrenergic agents (e.g., pseudoephedrine, oxymetazoline) reduce secretory activity

Treating the Underlying Cause

  • Medication-induced xerostomia: where clinically feasible, the prescribing physician may adjust the dose of the offending medication, substitute it with an alternative drug with a lower xerogenic profile, or change the timing of administration (e.g., taking the medication at night to minimize daytime symptoms). Patients should never discontinue prescribed medications without consulting their doctor.

  • Diabetes: optimizing glycemic control through appropriate insulin and/or oral hypoglycemic therapy, combined with adequate hydration, can meaningfully reduce diabetes-associated xerostomia.
